What a 3D areola tattoo is

A 3D areola tattoo uses shading and highlights to create the appearance of a raised nipple and textured areola on flat or reconstructed skin. It can be done on one side to match the other, or on both sides.

When you can start

Your skin needs to be fully healed. Most artists wait until your surgeon clears you, which is often several months after your final reconstruction surgery or radiation. Ask your surgical team if you're unsure; it's also fine to have a consultation early just to plan.

Step by step

  1. Consultation. You talk through your surgery history, timing and the look you want. The artist checks the skin, including any radiation changes or scarring.
  2. Design and color. Size and placement are drawn on and adjusted with you. Pigment is blended to match your skin tone or your other side.
  3. Tattooing. Reconstructed skin often has reduced sensation, so many clients feel little. Numbing is available where there's feeling.
  4. Healing. Expect some redness and light scabbing for a week or two. Keep it clean, wear soft loose clothing, and stay out of the sun.
  5. Touch-up. After healing, a follow-up session deepens color or refines detail if needed.

Will insurance cover it?

Federal law (the Women's Health and Cancer Rights Act) requires most health plans that cover mastectomy to cover breast reconstruction, and some plans reimburse nipple and areola tattooing as part of that. Coverage for a tattoo artist, rather than a medical office, varies, so ask your insurer before your appointment.
